As long as the financial report (audit/financial review) shows the relevant information pertaining the library, the document can be submitted in any form. It can be part of the larger financial report or the parts that are relevant to the library can be pulled out and submitted by itself as well. In either case, it is important that the "Independent Auditor's Report" is included in the document that is uploaded. The "Independent Auditor's Report" or similarly worded section of the report should contain a statement verifying who completed the audit or financial review as well as what type of review it was.
For example: "I have audited the accompanying financial statements of the XYZ Public Library (a nonprofit organization), which comprise the statement of financial position as of December 31, 2022, and the related statements of activities, functional expenses and cash flows for the year then ended, and the related notes to the financial statements." This statement is usually on a page with company letterhead and is signed, dated, etc.
